jordan f4cfd6c81f feat: complete M8 replication primitives + forage enhancements + docs
Milestone 8 (phases 1-4):
- Shard-aware WAL segment naming, BatchHeader v2, ShardRouter
- Transport trait, InProcessTransport, WalShipper, FollowerDb
